MOUNTAIN VIEW, Calif. (KTVU) - A major accident in the South Bay claimed a
life Friday morning. The driver of a Tesla Model X passed away after a
three-car collision on the Highway 101 in Mountain View.

That crash is also raising questions about how first responders handle
debris from heavily damaged electric cars. For three hours, a pair of Tesla
engineers performed a roadside tutorial in how to make it safe to remove a
heavily damaged battery from an electric car.

“We’re used to the regular vehicles. Now the batteries in these vehicles, we
don’t know what’s in them, so we’re learning because obviously it’s not
going to be the only crash,” said Art Montiel of the California Highway
Patrol station in Redwood City.

The burnt out Model X shell wound up sitting in the shoulder of southbound
101 near Shoreline Boulevard after the collision, according to Montiel.
Investigators said the Tesla’s driver ran into crash-cushions protecting the
concrete point where the freeway separates from the Highway 85 High
Occupancy Vehicle off-ramp.

Montiel says they have seen situations where drivers are traveling in the
carpool lane and see the split at the last second, realizing it’s not where
they want to go and changing their mind at the last second.

The impact sent the Tesla airborne, and when it landed on the pavement, it
was hit by two trailing cars – an Audi and a Mazda.

The Tesla’s driver died hours later at Stanford Hospital from injuries
suffered in the crash. A fire erupted at the scene, engulfing the Model X,
and exposing live wires feeding into its 70 percent charged battery.

“It may still be live. We’re not sure. Because of the extent of the damage
from the fire,” said Mountain View Fire battalion chief Zack Bond.

Tesla’s “First Responder” website has instructions in case one of its
vehicles catches fire. Those instructions state to use large amounts of
water – about 3,000 gallons -- to fight a high-voltage battery fire. It also
instructs ton use disconnects that come with each car to safely remove
battery components.

“The orange lines that are on the ground run high voltage through the
vehicle as we understand it. And because they’ve been severed completely,
and the disconnects are not available to us, we can’t be one-hundred percent
sure that the vehicle’s been made safe for tow personnel or first responders
to get close to,” said Bond.

Firefighters and CHP officers waited several hours for Tesla engineers to
arrive, and then begin the painstaking task of making sure the battery was
safe. They removed components one at a time, soaking each in a solution,
before discarding. The all clear was given shortly before the afternoon rush
began in earnest.

CHP officers stress this is a new thing for them and firefighters, as all
first responders learn the dos and don’ts associated with electric cars. The
drivers of the other two cars involved in this accident were not injured and
are cooperating with investigators. The CHP hasn’t released an official
cause but they say it doesn’t appear speed was a factor.

WASHINGTON (Reuters) - The U.S. National Transportation Safety Board said on
Sunday it was “unhappy” that electric car maker Tesla Inc made public
information about the crash of its Model X vehicle on Autopilot that killed
the driver last month.

The agency “needs the cooperation of Tesla to decode the data the vehicle
recorded,” NTSB spokesman Chris O’Neil said in a statement. “In each of our
investigations involving a Tesla vehicle, Tesla has been extremely
cooperative on assisting with the vehicle data.”

“However, the NTSB is unhappy with the release of investigative information
by Tesla,” he added.

A Tesla spokesperson declined to comment.

The board’s reaction to Tesla was first reported by the Washington Post
Sunday evening.

O’Neil was responding to Tesla’s announcement on Friday that the Tesla Model
X involved in the crash had activated its Autopilot system moments before
the March 23 mishap.

The driver, 38, died at a nearby hospital shortly after the vehicle hit a
concrete highway divider near Mountain View, California. The mishap involved
two other vehicles.

“The NTSB is looking into all aspects of this crash including the driver’s
previous concerns about the autopilot,” said O’Neil. “We will work to
determine the probable cause of the crash and our next update of information
about our investigation will likely be when we publish a preliminary report,
which generally occurs within a few weeks of completion of field work.”

Last week, the company said that a search of its service records did not
“find anything suggesting that the customer ever complained to Tesla about
the performance of Autopilot. There was a concern raised once about
navigation not working correctly, but Autopilot’s performance is unrelated
to navigation.”

In its announcement on Friday, the company said that shortly before the
crash, the vehicle’s “Autopilot was engaged with the adaptive cruise control
follow-distance set to minimum.”

Autopilot allows drivers to take their hands off the wheel for extended
periods under certain conditions. Tesla requires users to agree to keep
their hands on the wheel at all times before they can use Autopilot. Users,
however, routinely brag they can use the system to drive hands-free.

In its Friday statement, Tesla also said vehicle logs from the accident
showed no action had been taken by the driver right before the crash and
that he had received earlier warnings to put his hands on the wheel.

The statement did not say why the Autopilot system apparently did not detect
the concrete divider.

Well, considering a car is parked far more than it is driven, I would

A few facts that have been discovered about the crash:  Long approach to the center divider with no rumble strips and huge area not striped like a no mans land a lane wide seemingly encouraging drivers to change lanes at the last moment IMO. Two red cones in front of the divider as a visual marker.  The retractable end cover which retracts when hit was in the retracted position giving no help in a crash.  In other words the car hit a brick wall reinforced with a steel cage three feet wide and hundreds of feet long on end.  I wonder why Caltrans hadn't replaced the safety structure with at least sand or water barriers?  Lawrence Rhodes

This technology is a long way off, in my opinion. There are so many
tricky cases where human judgment still rules.

- snow on the road, obscuring lane markings
- wet pavement at night, glare from oncoming headlights obscuring lane
markings
- old paint markings
- extraneous paint, graffiti
- no lane markings, no fog line
- markings that don't align from one side of an intersection to another
- police directing traffic
- interpreting one-way street signs (maybe map data can help with this).

I'm sure there's much more. Great technology, but "autopilot" is easily
misinterpreted to mean autonomous. Even in aviation, "autopilot" is not
autonomous. You still need to monitor the gages, watch the air space for
other air traffic, and in particular, handle landing.

Flashback to a classic Wile E. Coyote cartoon, where he paints stripes on the road to direct the Road Runner into a granite wall where he had painted a tunnel. The Road Runner of course runs into the tunnel that isn't there.

How long before some "coyote" does the same thing in real life, directing a Tesla into a wall?

I believe Boeing has been working on landings, but relying on precise
GPS rather than stripes on the landing strip.
